Please pre-arrange for someone to take you home and care for you on discharge.
You may discharge yourself from hospital or from treatment at any time. However, you do so at your own risk and the hospital will ask you to sign a self-discharge form.

Our Community Liaison staff are available to help you plan your discharge, if you have complex requirements. They can help arrange community support if required.
